A Third Career Built on Service

Dr. Corie Rowe's path to dentistry is unlike any other. Dentistry is, in his own words, his third career. Before graduating from Tufts University School of Dental Medicine, he worked as an environmental justice advocate, helping low-income Boston communities address self-identified environmental issues, and then as a network engineer servicing small and medium-sized businesses.

The common thread running through all three careers? Being of service. That philosophy is the foundation of everything at Comfort Family Dental at 2036 W 95th Street in Beverly.

Interestingly, Dr. Rowe never intended to become a dentist. In 1992, his mentor suggested it as a career path when he expressed a desire to work with science to help others. He dismissed it, largely due to a painful childhood dental experience in Jamaica. That personal history became the driving force behind his commitment to making dental care genuinely comfortable for every patient who walks through his door.

Dr. Rowe is the author of a peer-reviewed study on preventive dental treatments for young African-American children, published in the Journal of Public Health Dentistry. The Link Between Gum Disease and Alzheimer’s Disease

What a decade of peer-reviewed research tells us about the connection between chronic periodontal disease and one of the most feared conditions of aging, and what Beverly, Chicago patients can do about it today.

Key finding: A 2023 systematic review published in Cureus (Bouziane et al.) found that patients with periodontal disease present a significantly higher risk of Alzheimer’s disease compared to periodontally healthy individuals. A separate 2024 narrative review in Frontiers in Aging Neuroscience confirmed the risk of developing Alzheimer’s may double within a decade following a diagnosis of chronic periodontitis.

What Is the Connection?

For decades, dentistry and neurology were treated as entirely separate fields. The mouth was the mouth. The brain was the brain. That division is crumbling under the weight of accumulating evidence.

Alzheimer’s disease is the most common form of dementia, affecting an estimated 6.7 million Americans over the age of 65, and representing the fifth leading cause of death in that age group. It is defined neuropathologically by the accumulation of beta-amyloid plaques, neurofibrillary tangles of hyperphosphorylated tau protein, and chronic neuroinflammation driven by activated microglia and pro-inflammatory cytokines including IL-1β, IL-6, and TNF-α.

Periodontal disease is a chronic inflammatory condition affecting the supporting structures of the teeth, characterized by dysbiosis of the oral microbiome, bacterial biofilm formation, immune dysregulation, and progressive destruction of gum tissue and alveolar bone. What researchers have discovered is that these two conditions share more than risk factors. They may share a biological pathway.

The Smoking Gun: Porphyromonas Gingivalis in the Brain

The most striking finding in recent years has been the detection of Porphyromonas gingivalis (P. gingivalis), the primary pathogen in chronic periodontitis, in the brain tissue of deceased Alzheimer’s patients. P. gingivalis produces toxic proteases called gingipains that are neurotoxic and appear capable of breaching the blood-brain barrier.

A 2024 narrative review in MDPI International Journal of Molecular Sciences (Bednarski et al.) documented that brains of individuals with Alzheimer’s disease have substantiated the existence of microorganisms related to periodontitis, especially P. gingivalis, which produces neurotoxic gingipains and may have the capability to breach the blood-brain barrier.

Additional periodontal pathogens including Tannerella forsythia and Treponema denticola have also been identified as organisms capable of persisting intracellularly, evading host immune defenses, and disseminating via the bloodstream to distant sites including brain tissue.

The Inflammatory Cascade Pathway

Even without direct bacterial invasion of brain tissue, chronic gum disease drives a systemic inflammatory response that can cross the blood-brain barrier. Periodontal infection triggers the sustained production of pro-inflammatory cytokines (IL-1β, TNF-α, IL-6) and prostaglandin E2, which enter the bloodstream and circulate throughout the body.

A 2024 review published in MDPI Dentistry Journal (Kamer et al.) found that systemic inflammation can exert an effect across the blood-brain barrier, activating microglia. Activated microglia can result in amyloid-β accumulation, neuronal death, and cognitive impairment. Elevated levels of pro-inflammatory cytokines such as TNF-α and IL-1β have been observed in the saliva and serum of individuals with both Alzheimer’s and periodontitis simultaneously, suggesting an overlapping inflammatory burden.

Two Primary Pathways Researchers Have Identified

Pathway 1
Microbial Invasion

Periodontal bacteria (primarily P. gingivalis) and their toxic byproducts enter the bloodstream, cross the blood-brain barrier, activate microglia, and directly promote the accumulation of beta-amyloid plaques and tau tangles characteristic of Alzheimer’s disease.

Pathway 2
Inflammatory Cascade

Chronic periodontal inflammation drives sustained systemic elevation of IL-1β, IL-6, and TNF-α. These cytokines cross the blood-brain barrier, trigger neuroinflammation, accelerate neuronal damage, and exacerbate the progression of Alzheimer’s disease pathology.

Clinical Studies: The Human Evidence

The evidence is not limited to laboratory findings. Multiple large-scale clinical and epidemiological studies have examined this relationship directly:

  • Taiwan National Health Insurance Database Study (Chen et al., 2017): Analysis of a large national database showed that chronic periodontitis increased the prevalence of Alzheimer’s disease, with a more significant impact observed in older populations. The authors concluded that preventive measures and routine periodontal examinations are critical for at-risk groups.
  • Six-Fold Cognitive Decline Finding (Ide et al., 2016, Journal of Alzheimer’s Disease): A study of 60 non-smoking subjects with mild to moderate dementia found that the presence of periodontitis was associated with a six-fold increase in the rate of cognitive decline over a six-month period. The authors concluded the decline may be mediated through systemic inflammation.
  • Systematic Review and Meta-Analysis (Bouziane et al., Cureus, 2023): A PRISMA-guideline systematic review analyzing cross-sectional, cohort, and case-control studies from five major databases (PubMed, ScienceDirect, EBSCO, Web of Science, Scopus) concluded that patients with periodontal disease present a significantly higher risk of Alzheimer’s disease compared to periodontally healthy individuals.
  • Scoping Review, Frontiers in Aging Neuroscience (2025): A comprehensive scoping review noted that the risk of developing Alzheimer’s may double within a decade of a periodontitis diagnosis, and emphasized the importance of periodontal health management as part of dementia prevention strategy.

What This Means for You

The causal relationship between gum disease and Alzheimer’s has not been definitively established, and researchers are appropriately cautious about claiming one directly causes the other. What is clear is that a significant association exists, that biologically plausible mechanisms have been identified, and that treating gum disease carries no downside risk whatsoever.

Key finding: A 2023 systematic review and meta-analysis of longitudinal studies (National Association of Dental Plans / Sage Journals) found that poor periodontal health and tooth loss appear to independently increase the risk of both cognitive decline and dementia, with risk of diminished cognitive function increasing with incremental numbers of teeth lost.

Dementia in America: The Scale of the Problem

Dementia is an umbrella term for a group of conditions characterized by progressive decline in memory, language, problem-solving, and other cognitive functions severe enough to interfere with daily life. Alzheimer’s disease accounts for approximately 60 to 80 percent of dementia cases. The remaining cases include vascular dementia, Lewy body dementia, and frontotemporal dementia.

The economic and human cost is staggering. According to the Alzheimer’s Association, more than 55 million people worldwide live with dementia, with nearly 10 million new cases diagnosed each year. While age and genetics are the strongest risk factors, researchers have increasingly focused on identifying modifiable contributors, behaviors and conditions that can be changed or treated to reduce risk.

Oral health has emerged as one of those modifiable factors, and the evidence connecting periodontal disease to dementia risk is both surprising in its strength and logical in its mechanism.

Tooth Loss as a Cognitive Risk Factor

One of the most consistent findings in oral-systemic health research is the independent association between tooth loss and cognitive decline. A 2023 systematic review and meta-analysis published in Sage Journals found that moderate-quality evidence indicated tooth loss was independently associated with cognitive impairment and dementia, and that the risk of diminished cognitive function increased with each additional tooth lost.

A 2023 prospective analysis examining diabetes, edentulism (complete tooth loss), and cognitive decline over 12 years found that the co-occurrence of diabetes and complete tooth loss led to significantly worse cognitive function and faster cognitive decline in adults aged 65 to 74, compared to individuals with either condition alone.

Gum Disease and Systemic Inflammation: Your Mouth Is Not an Island

The Mouth as a Window to Systemic Health

For most of the twentieth century, dentistry operated in isolation from the rest of medicine. The mouth was thought of as a separate system, its diseases local problems with local consequences. That model has been fundamentally overturned.

We now know that the mouth is the entry point of the body, and that chronic oral infection creates a sustained inflammatory burden that circulates systemically. The periodontal pocket, the space between your teeth and gums in active gum disease, is a wound. It is ulcerated tissue in direct contact with a dense community of pathogenic bacteria. Across a full-mouth infection, that wound may total the surface area of an adult palm.

A wound of that size, anywhere else on the body, would be treated aggressively and immediately. In the mouth, because it causes no obvious bleeding visible to the patient, it is often ignored for years or decades.

The Biology of Periodontal Inflammation

Periodontal disease begins as gingivitis, inflammation of the gum tissue in response to bacterial biofilm accumulation. If left untreated, it progresses to periodontitis, in which the inflammatory response begins destroying the connective tissue and bone supporting the teeth.

At the cellular level, this process involves the infiltration of neutrophils, macrophages, and lymphocytes into periodontal tissues. These immune cells produce pro-inflammatory mediators including interleukin-1 beta (IL-1β), tumor necrosis factor alpha (TNF-α), interleukin-6 (IL-6), prostaglandin E2, reactive oxygen species, and matrix metalloproteinases. These are not gentle signals. They are aggressive immune weapons designed to destroy bacteria that, when chronically elevated, damage the body’s own tissues.

In active periodontitis, these mediators enter the bloodstream in sustained, elevated concentrations. The result is a state of chronic low-grade systemic inflammation that research has now linked to a remarkable range of serious diseases beyond the teeth and gums.

Systemic Conditions Linked to Periodontal Inflammation

Cardiovascular Disease and Stroke

The American Heart Association has acknowledged an independent association between periodontal disease and cardiovascular disease. Systemic IL-6 and TNF-α elevation driven by periodontal infection promotes atherosclerosis, the buildup of plaque in arterial walls. Periodontal bacteria including P. gingivalis have been identified in atherosclerotic plaque. Studies suggest periodontal disease patients face meaningfully elevated risk for heart attack and stroke compared to periodontally healthy individuals.

The relationship between diabetes and periodontal disease is bidirectional and well-established. Diabetes impairs immune response, creating conditions favorable for periodontal pathogen overgrowth. The 2023 prospective study on diabetes and edentulism found that patients with both conditions experienced significantly accelerated cognitive decline, illustrating how these systemic conditions compound each other.

Rheumatoid arthritis and periodontitis share remarkably similar inflammatory mechanisms, including elevated IL-1β, TNF-α, and matrix metalloproteinase activity driving tissue destruction. P. gingivalis has been specifically implicated in generating citrullinated proteins that may trigger autoimmune responses characteristic of RA. Multiple studies have found higher rates of periodontitis in RA patients and vice versa.
